FIM Assessments

FIM Assessments
Standardised measurement of functional independence to support rehabilitation planning and outcome tracking.
Insight Rehab provides FIM (Functional Independence Measure) Assessments to objectively evaluate a person’s level of independence across key areas of daily functioning.
These assessments are widely used in rehabilitation settings to support clinical decision-making, track progress over time, and inform funding, discharge planning, and ongoing care needs.
What is a FIM Assessment? A FIM Assessment is a structured, evidence-based tool used to measure a person’s functional independence in everyday activities. It provides a consistent way to assess how much assistance a person requires across physical, cognitive, and communication domains. The assessment helps answer key rehabilitation questions such as: •What level of support does the person currently require? •How has function changed over time? •What level of independence is realistically achievable? •What supports or services are required to maintain safety and function?
Areas Assessed FIM Assessments typically evaluate independence across key domains, including: •Self-Care •Eating, grooming, bathing, dressing, toileting •Mobility •Transfers (bed, chair, toilet, shower) •Walking, wheelchair use, stairs Continence •Bladder and bowel management Communication •Expression and comprehension of information Cognition •Memory •Problem-solving •Social interaction and safety awareness Each item is scored using a standardised rating system to reflect the level of assistance required.
When is a FIM Assessment Used? FIM Assessments are commonly used in: •Rehabilitation programs (inpatient and community-based) •Post-injury or post-surgical recovery •Neurological conditions (e.g. stroke, brain injury) •Complex or long-term disability cases •Discharge planning and transition planning •Insurance and compensation schemes •Functional outcome monitoring over time
What the Assessment Involves Our Occupational Therapists complete a structured evaluation that may include: •Clinical interview and background review •Observation of functional tasks •Assessment of physical and cognitive abilities •Review of care requirements and support needs •Collaboration with treating teams where required Where appropriate, assessments are conducted in real-world environments such as home, hospital, or rehabilitation settings.
Outcomes & Reporting Following assessment, we provide a clear, structured report outlining: •FIM scoring across all domains •Level of independence and assistance required •Functional strengths and limitations •Progress compared to previous assessments (if applicable) •Recommendations for rehabilitation, supports, or care planning Reports are designed to support: •Rehabilitation goal setting •Funding and service planning •Discharge and transition decisions •Insurance and case management requirements
