Insight Rehab provides Occupational Therapy-led hospital discharge planning services to support safe, timely, and well-coordinated transitions from hospital to home, rehabilitation, or supported accommodation.
We work closely with hospital teams, patients, families, and community services to ensure functional needs are identified and appropriate supports are in place prior to discharge.
What is Discharge Planning? Discharge planning involves assessing a person’s functional abilities and support needs to ensure a safe and supported transition out of hospital care. The goal is to reduce readmission risk, improve safety, and support continuity of care in the community.
What We Assess We review a person’s functional status and discharge needs, including: •Mobility and transfer ability •Activities of daily living (ADLs) •Cognitive and communication function (where relevant) •Home environment suitability •Equipment and assistive technology requirements •Care and support needs post-discharge •Safety risks and barriers to independence
When is Discharge Planning Required? Discharge planning is commonly required for: •Post-surgical recovery •Stroke or neurological conditions •Orthopaedic injuries (e.g. fractures, joint replacements) •Complex medical conditions affecting mobility or cognition •Older adults at risk of functional decline or falls •Patients requiring ongoing care or community support
