Home Assessments

Home Assessments
Supporting safe, independent living through evidence-based home environment assessment and recommendations.
Insight Rehab provides Occupational Therapy Home Assessments to evaluate a person’s ability to safely function within their home environment and identify strategies, supports, or modifications to improve safety, independence, and quality of life.
These assessments are commonly used following injury, illness, surgery, or functional decline where there are concerns about safety, mobility, or daily living needs.
What is a Home Assessment? An Occupational Therapy Home Assessment is a structured evaluation of a person’s home environment and functional abilities within that setting. The purpose is to identify barriers to independence and recommend practical solutions that support safe and effective daily living.
What We Assess We evaluate how a person functions within their home, including: •Mobility and access within the home •Bathroom, kitchen, and bedroom safety and usability •Falls risk and environmental hazards •Ability to complete activities of daily living (ADLs) •Equipment and assistive technology needs •Care and support requirements •Entry and exit accessibility Assessments are completed in the home environment to ensure recommendations are practical and tailored to real-world conditions.
Possible Recommendations Following assessment, we may recommend: •Minor home modifications (e.g. grab rails, non-slip solutions) •Major home modifications (e.g. ramps, bathroom redesign) •Assistive equipment and aids •Mobility support strategies •Personal care supports •Functional training or rehabilitation input •Referrals to additional services where required All recommendations are based on safety, function, and long-term independence.
When is a Home Assessment Needed? Home assessments may be appropriate following: •Hospital discharge planning •Injury or surgical recovery •Neurological conditions (e.g. stroke, brain injury) •Falls or mobility decline •Chronic health conditions affecting function •Changes in care needs or living situation
